Co-op Preschool and Kinder Bridge Application Process
Co-op Preschool Application Forms & Registration Fee – Please pick up and complete the student application form available in the Early Childhood Center’s preschool office located in Kid Kountry. Only completed application forms will be considered. A non-refundable registration fee of $150 must accompany each application. Applications received after July 1 require a non-refundable registration fee of $200. The enrollment packet you receive after completing the application form must be completed and submitted to the preschool office in order for final acceptance to be granted. NOTE: All application forms must be picked up in the preschool office.

Parent & Child Orientation Schedule
Tuesday 8/28/07
- Tuesday & Thursday AM classes: 9:00 - 10:15 AM
- All PM classes: 11:00 - 12:15 PM
Wednesday 8/29/07
- Mon/Wed/Fri & Monday through Thursday AM classes: 9:00 - 10:15 AM
Wait Pool – If coop classes are full, you may submit a completed Co-op Wait Pool form accompanied with a fee of $25. This fee is applied towards the $150 or $200 registration fee as soon as there is an opening. Current Cooperative Preschool registered students may submit a Wait Pool form for alternate classes with no additional fee.
May Tuition Deposit – Tuition deposit for the last month (May 2008) of school will be processed through the FACTS Tuition Management Program either July 5 or July 20. Your statement for the May tuition deposit will be mailed to you on July 1. Registrations submitted after June 29 must be accompanied with the $100 non-refundable registration fee and the tuition deposit for the last month (May 2008) of school.
Tuition Assistance – You may request a Tuition Assistance Application from the school's business office.
Extended Care Preschool Application Process
Extended Care Preschool Application/Wait Pool Forms & Registration
Fee – Please pick up and complete the student Registration/Wait Pool
form available in the Early Childhood Center’s preschool office located
in Kid Kountry. Only completed application forms will be

Theseitems must accompany the completed application:
- A non-refundable registration fee of $150
- Current months tuition
- Lunch fee (if applicable)

Wait Pool – If classes are full, you may submit a completed Registration/Wait Pool form accompanied with a fee of $25. This fee is applied towards the $150 registration fee as soon as there is an opening. Families will be contacted as soon as an opening is available.
Annual Renewals – These items are due each year on the anniversary of student's starting month:
- Updated State of AZ Emergency & Health Form
- Updated Joy Preschool Emergency & Health Card
- Updated Child Information Form
- Updated Over the Counter Form
- Annual renewal fee of $100
Tuition – Tuition for Extended Care Preschool will be processed through the FACTS Tuition Management Program either on the 5th or 20th of each month.
Tuition Assistance – You may request a Tuition Assistance Application from the school's business office.
Kindergarten Application Process
All items below must be completed and submitted to the Early Childhood Center office before a final acceptance is granted.

The Application – Complete the new student application or re-enrollment form. Be sure all areas that require a signature are addressed (N/A indicates non-applicable). A recent photograph of applicant must be attached to new student application.
- Returning students: please keep the first page and submit the second page including all returning students on application page.
- New students: A non-refundable registration fee of $400 (per child) must be submitted with the application. This fee is non-refundable unless it is determined that Joy Christian School cannot appropriately accommodate the academic needs of your child. You will be informed of acceptance prior to the beginning of the school year.
Previous School Records and Letters of Recommendation – All incoming
kindergarten students must provide a Preschool Recommendation Form. All
new incoming students must provide a Parental Involvement and Student
Discipline Survey.
Joy Christian School Required Documents:
- Emergency Information card (blue form). This form includes your emergency contacts, shot records and any details medically related to the child.
- Private School Affidavit. This form informs the County/State where the child is attending school.
State/ACSI Required Documents:
- Immunizations (obtained from physician or parents copy)
- Birth Certificate/Passport
Financial Requirements:
- FACT tuition forms (questions directed to bookkeeping)
- ACSTO scholarship award application (if applicable)
- First month’s tuition
Interview – You will be contacted to schedule an interview with the
Principal, if one is not scheduled during a tour. Interviews with the
principal are required for all new families.
Testing/Assessments – Kindergarten assessments – all new kindergarten parents will be contacted to schedule assessments in June prior to the new school year. New kindergarten students enrolling mid-year will be assessed from the most current Dibels assessments.
